Plan With Care provides general information about burial benefits and private final expense insurance. It is not affiliated with or endorsed by the Social Security Administration, the Department of Veterans Affairs, or another government agency. Content on this site is not a government-benefit determination, an insurance quote, or a promise of coverage. Benefit rules and amounts may change. Confirm government benefits with the agency that administers them.
Review requests and insurance
You may request a free review through the form. Optaris, LLC, a licensed insurance agency, may contact you as described next to the form and may offer private final expense insurance. You are not required to buy anything. Insurance availability, rates, eligibility, waiting periods, and coverage depend on the insurer, policy, your state, age, and health. The agent does not determine eligibility for government benefits.
Phone, email, and text contact
By clicking “Request My Free Review” below the form disclosure, you electronically agree that Optaris, LLC may contact you by phone, email, and recurring automated text about your request and private final expense insurance options. The SMS Terms explain the sender, purpose, message frequency, possible charges, and how to stop texts. Consent is not required to buy insurance. To request a review without automated calls or texts, email support@plan-with-care.com. You may revoke contact permission by emailing that address or telling the agent; you may also reply STOP to a text.

Use of the site
Use the site only for lawful purposes. Do not submit another person's contact details without permission, interfere with the site's operation, or misuse its content. External links are provided for convenience; their operators control their own content and policies.
