Plan With Care is operated by Leadbolt at 710 NE 1st Ave, Apt 4808, Miami, FL 33132. This policy explains how we handle information collected through this website. Questions and privacy requests may be sent to support@plan-with-care.com.

Information we collect

When you request a review, we collect the first and last name, email address, phone number, ZIP code, city, and state you enter. We record your form submission, the consent wording and button shown to you, the date and page of your request, and referral or campaign information associated with your visit. We also record technical details such as IP address and browser information to document consent, operate the site, and help prevent fraud.

The request page uses TrustedForm, a service from ActiveProspect, Inc., to document consent. While you are on that page, TrustedForm records how the page appeared and how you interacted with it, including what you enter in the form, and creates a certificate of your request. The certificate link is sent with your request to the agency and may be used to verify your consent.

The site stores referral and campaign parameters in browser session storage during your visit. Our hosting provider may process request logs and device information needed to deliver and protect the site.

How we use information

A licensed insurance agent may discuss private final expense insurance. Government agencies, rather than Plan With Care or an insurance agent, determine eligibility for government benefits.

Who receives information

We share completed review requests with Optaris, LLC, the licensed insurance agency listed on our Insurance Partners page. Leadbolt may receive compensation for that referral. We also use hosting, security, consent-documentation, and lead-delivery service providers that process information for us or the agency. We may disclose information when required by law or in a business transfer.

The form submission includes consent for the named SMS program. It is not permission for other agencies to send marketing texts.

Contact choices

You can tell an agent to stop calling, reply STOP to a text, or email support@plan-with-care.com. You may request a review through that email without agreeing to automated calls or texts. Text-message consent can be withdrawn. Commercial emails include an opt-out method where required. We pass relevant opt-out requests to the agency and service providers that handle contact on our behalf.

Do Not Sell or Share My Personal Information

Sharing a request with an insurance agency for compensation may be considered a “sale” or “sharing” under some state privacy laws. To request an opt-out, email support@plan-with-care.com with the subject “Do Not Sell or Share My Information.” Include enough information for us to locate your request, such as the email address or phone number you submitted. We may need to verify your identity before acting.

Your privacy rights

Depending on where you live, you may have rights to learn what personal information we hold, request correction or deletion, receive a copy, or opt out of certain sharing or targeted advertising. You may also use an authorized agent where applicable. Send a request to support@plan-with-care.com; we will respond as required by applicable law and explain any available appeal process if a request is denied.

Retention and security

We retain review and consent records for as long as reasonably needed to respond to requests, document consent and opt-outs, meet legal obligations, and resolve disputes. We use reasonable safeguards and limit access to people and providers who need the information for those purposes. No internet transmission or storage method is completely secure.

Children and external links

This site is intended for adults and does not knowingly collect personal information from children under 13. Links to government and other external websites lead to services with their own privacy practices.
